Physician Information
Purpose:
The ‘Physician Information’ window allows you to record and access the physician or other health care provider’s (HCP) referral information. This information is for your reference and used in preparing claim records and communications.
Function:
This button is found on the main Control Panel. Here you enter the appropriate HCP data into the data fields. This information will be used in other forms including the Session Notes and Physician’s Diagnosis. Click the ‘Referrals with Diagnosis’ button to open the ‘View/Edit Diagnosis’ window to access diagnosis records for the patients referred from this HCP.
Click on ‘Write Them a Letter button to open a preformatted letter in MS Word with this HCP’s name and address already included. Click on the ‘Patient List’ report button to view a list of patients associated with this HCP.
Notes:
It is no longer required to include a UPIN number but you must have an NPI number for each HCP if you plan to prepare a claim. You may find the NPI number of an individual HCP by going to this website:
