
Patient Session Form
Purpose:
It’s in the Session notes where you track all the information you need to document and track data for the patient’s visit. This window provides access to tools and resources for recording and referencing each element of the nutrition care process including evidence-based guidelines and creating tailored educational handouts.
Function:
Confirm that the date recorded is the date of the contact. It will automatically insert today’s date. Using the tab key, move to the next field and type in the start time. Be sure to include AM or PM and make sure that the time is earlier than the stop time. Tab to the stop time and enter a value that is after the start time. When you tab out of this field, it will calculate the total time. After selecting a CPT code the number of units will be calculated and appear in the window.
Select the RD (Therapist) and select the Guidelines. The RD may be already selected if you already have entered the RD in the patient profile. Click the pencil button next to the guidelines, to paste a template of notes into the comments box. This template is associated with the selected guideline topic. See Guidelines Management to learn more on how to edit these template notes.
Choose the physician’s Diagnosis. See Diagnosis Form for more information on setting this up. Then complete your notes in the comments section. When you are finished, you can check the spelling of your notes but clicking the ‘ABC’ button at the bottom of the comments box.
When you click on the Session button, under the Reports header, you will preview and print your chart notes. Many of the tools and feature of the MNT Assistant are accessed from the session notes window. Notes:
When you want to record a ‘No Show’ and then charge for it, just create a new session for that date then create a billing record with the No-show charges.
