
Patient Profile Form
Purpose:
This window is the heart of the patient record. It contains all the patient identity and contact information along with personal insurance and payer data. From this window you may access all contact records including billing and clinical service reports.
Function:
When starting a new record immediately enter the patients name and gender. This initiates the new record. Click on the tabs to access all available information stored on this form. After completing the patient contact information you may click the ‘Export Patient to Outlook’ button to create a record in Microsoft Outlook (a program in MS Office).
When you enter the date of birth the window automatically computes the age. After selecting the primary care physician, Primary MNT Therapist or insurance carrier you can access their contact information but clicking the more information buttons marked with a red question mark (?).
Record your patient’s MV Professional Nutrition Suite user name in the field labeled “MV Pro Online User Name”. This user name will allow you to conveniently download completed profile data stored on your patient’s MV Pro online account.
Click ‘New Session’ or select a session date and click ‘Open Session’ to go to the session information window. Data for each patient visit is recorded in the session notes. To delete a session, you must select a session and then click the session delete button which is adjacent to the ‘New Session’ button.
Record other contacts with the patient by clicking on ‘New Contact’ or click ‘Open Contact’ after selecting a contact date. Here you record contacts such as emails, letters, and phone communications. It links with other office programs including MS Word for letters and MS Outlook for emails.
Notes:
By changing the Chart status code, you may omit this patient from all group reports and lists. Just change the code from a ‘0’ to a ‘1’. It shows red when the value is ‘1’.
Enter the ‘Primary Policy Holder Other than Patient’ data when a patient is on another person’s insurance plan. This is often the case for a child. The parent’s information would be listed as the primary policy holder.
