MNT Therapist Information Form
Purpose:
The MNT Therapist Information form provides a place to record the identity and contact information for all users of the MNT Assistant. Here is where you access Biller information and print a list of all patients seen by the therapist identified in this window.
Function:
This button is found on the main Control Panel. Here you record the Registered Dietitian’s user password and contact information including the NPI number and other identifiers. However, since the NPI number is now the main number required by all health care payers, it is not necessary to include the old Medicare PIN or Social Security Number. Some payers may insist on asking you to include their own identifier. In this case, click on ‘Payer PINs’ to record the payer identifier associated with the assigning payer or insurance company.
When changing the user password you need to first enter the current password. If you have forgotten your password, please call support for assistance (503) 766-2471.
The ‘Office Name and Location’ will be printed on the header of your session notes reports.
All clerical or assistant staff information will also be recorded in this window. Use the navigation tool at the lower left corner of the window to move from one record to the next. Set up all users shortly after installation.
Click the ‘Close Form’ button when you want to leave this form.
Notes:
Don’t forget to record the Biller Information even if it is the same. Click on the Biller Information button to record you organization’s information.
Mark ‘Accepts Assignment’ in this window if you are willing to accept the assigned payer payments. This mark will appear on section #27 of the CMS 1500 form. .
