Claims Editor Form

Purpose:
The Claims Editor form will help you build a claim record in preparation for printing a claim on a CMS 1500 form or sending the claim information via electronic claims processing.

Function:
After clicking the ‘Claims Editor’ button the program will gather most of the necessary information for you to prepare a claim. It’s important that you complete all parts of the patient record that may be involved in the claim before you open the claims editor. Be sure the insurance company has been selected, proper ID numbers are entered and ICD9 codes, CPT codes and time values are correct.

After opening the Claims Editor, most of the field will have been entered if data was available. Check through the form by clicking the tabs at the top of the form and notice values in the fields marked in Red. Check these for accuracy.

When you believe you have all the data, click the ‘Process Data’ button. Upon successful completion of data processing, a date and time is recorded at the top. If you want to process the data again, delete the process date by clicking the delete process date button. When finished, you may now be ready to export or print the claim.

On the session notes window, clicking ‘CMS 1500’ report button will open MS Word. Click ‘Yes’ to import the claims data. Two documents will open. If you do not see the background of the CMS 1500 form go to MS Word Preferences and select ‘View hidden text.’ Use a standard CMS 1500 form in your printer and print the Word document to that form. The background should not print; only the text should print.

The claim will be marked as exported when you either print the CMS 1500 form or export the data with the Billing and Claims Utility Form. If you want to print or export again, first delete the export date on the claim.

Notes:
Line 33, Biller Provider Information, comes from the MNT Biller Information Form. This is the address you want the payer to send the check. If this is a different address then where the patient visit was provided then you will need to enter that address in line 32. This may be the Therapist’s address. If so, just click on the pencil button under line 32 and the MNT Therapist’s address will be pasted into this section.
