The Billing Record Form

Purpose:
The purpose of the billing record is to view and edit a list of your billing records. It is designed to help you reconcile your billing records.

Function:
To get started, type in a date range for the records you want to view or edit. The format of the date needs to be mm/dd/yyyy. The date range is required to see any records. With just a date range, click on ‘Billing Record’ and you will see a list of all billing records. You may segment this list by indicating a specific therapist and/or patient before opening this list.

The list shows the Dietitian’s name, patient’s name, contact or session date, amount due, Paid indicator, amount received from insurance, amount received from the patient (Copay) and any payouts you may have made on behalf of this visit. The ‘Notes’ line allows you to record accounting information you want to remember.

Notes:
The amount due is often your standard billing charge. What you receive may be less than what you have billed. If the account has been satisfied, mark ‘Paid’ when the session is considered ‘Paid’. The ‘Payouts’ line allows you to track fees you have paid out to a therapists. This allows you to calculate actual income (‘Received’) from a session.
