MNT Biller Information Form
Purpose:
The Biller/Group information form provides a place to record the contact information of your main office or where you want payers or insurance companies to send their payments. It also provides a place to record your MV Professional Online App Code and define user privileges.
Function:
Access this form from the MNT Therapist Information window. The Organization Name, NPI and contact information recorded here will appear on the super bill, electronic claims and the CMS 1500 form section #33 (see also Claims Editor). The SSN or EIN will appear on section #25.
An image such as a logo may be stored here. This image will be printed on session notes or the super bill along with your contact information.
The MV Professional Online App Code is required for use with the MV Professional Nutrition Suite. It is required to download encrypted patient data from the secure online portal (a single code is assigned with each standard or group subscription to MVP online).
Here you will be able to select user privileges for each user name. To make changes, select a user name. By default, the program allows anyone to type a new name on the login screen and create a new user. But, if you select “Require a user name to log in to MNT Assistant” and save it to any single user name, then it will only allow users in the list to log in. Select a user name and then check any box corresponding to the restriction you want to impose. Leaving all boxes un-checked will allow the user to have all edit and access privileges.
By clicking the ‘Logged User Report’ and selecting a user and date range, you’ll create a list of patients with dates and time records were opened by this user.
Notes:
To maintain the highest security we suggest you mark “Require a user name to log in to MNT Assistant” with your user name, soon after you install the MNT Assistant. If you forget your username and are unable to access the MNT Assistant, please call support at 503-766-2471.
The Organization Name appears on the header of the super bill.
